| Abstract | Background: Virtual reality (VR) can be used to create a realistic and complex scenery that at the same time is fully controllable. VR therefore enables the investigation of symptoms and mechanisms of mental disorders such as attention deficit hyperactivity disorder (ADHD) under realistic yet standardized conditions. Methods: In this review, the currently existing research on the use of VR for the investigation of ADHD is presented based on selected areas of application. For each application area, a brief summary of the state of research and the degree of maturity achieved is given. Results and Discussion: Most studies to date have been conducted on virtual classroom and road traffic scenarios. The classroom scenarios provide good evidence that VR can be used to develop neuropsychological test environments that make it possible to distinguish individuals with and without ADHD and medicated from non-medicated conditions. The road traffic scenarios in turn provide initial indications that they appear to be well suited for investigating the driving performance and road crossing behavior of people with ADHD. Although the VR scenarios presented show promising results, further validation studies need to be carried out and approval processes completed before the VR-based tests can be used in regular patient care.
