| Abstract | Background: Attention-deficit/hyperactivity disorder (ADHD) is diagnosed more frequently in males than in females. This review explores different factors and their contribution to the evaluation and treatment of female ADHD. Current diagnostic tools, predominantly validated on male samples, often fail to capture typical female symptoms. Methods: This review highlights core aspects of female ADHD that are particularly relevant in research and clinical practice. These key aspects were investigated in detail based on the latest scientific literature. Discussion: Gender-specific differences in symptoms make early diagnosis and adequate treatment more difficult in women. Less overt symptoms, masking behaviors, and coping mechanisms, as well as hormonal and neurotransmitter changes, obscure ADHD symptoms. Adult informants are less reliable in recognizing ADHD in females, contributing to biased symptom perception. Gender-sensitive diagnostic and therapeutic instruments are crucial. Until then, greater emphasis should be placed on self-reports to reduce diagnostic errors caused by biased perspectives. Adult ADHD criteria should be applied earlier for females to better align with their symptoms. Since school-age girls typically have more developed executive functions (EF) than boys, they are better able to use coping strategies to mask ADHD symptoms. Good academic performance should not be viewed as evidence against ADHD in females. Neurobiological factors should be considered. (ZPID). |
