AbstractInvestigated the consequences of institutional care in infants from Tanzania. 35 children (aged 8-15 years) who were institutionalized at birth to four years of age were compared to a matched group of 35 children who were institutionalized at five to 14 years of age (aged 5-14 years). Adverse childhood experiences were examined over the course of the children's entire lives - in their family of origin and in institutional care - using the Maltreatment and Abuse Chronology of Exposure - Pediatric Interview (pedMACE). Mental health problems at primary school age (depressive symptoms, aggressive behavior, and internalizing and externalizing problems) were assessed using the Children's Depression Inventory (CDI), the Reactive-Proactive Questionnaire (RPQ), and the Strengths and Difficulties Questionnaire (SDQ). Results show that early institutionalized children reported more adverse experiences during their time in institutional care and a greater variety of mental health problems than did late institutionalized children. Moreover, maltreatment in institutional care was positively related to mental-health problems only in early institutionalized children. It is concluded that adverse experiences in institutional care play an important role for early institutionalized children, who need special care from adequately educated caregivers. Therefore, it is argued that training concepts focusing on the needs of the youngest children have to be developed, tested, and established. Furthermore, policy changes for orphanages are suggested for countries such as Tanzania, to ensure an adequate standard of quality in childcare. (ZPID).
