Abstract	This article presents the author's observations on a clinical extract which describes a child psychotherapist working with Maya, a seriously ill child, in a hospital. The author said that the hospital setting raises the question of what it is like to apply child psychotherapists' thinking within a hospital setting, and how they adapt the way they work, including relating to family members, medical and auxiliary staff. The author observes that generally the psychotherapist in this extract manages to maintain his/her thinking and feeling capacity in an extremely difficult situation. She concludes that the psychotherapist's work, then is an attempt to bear the irreconcilable tension between the destructiveness of a serious illness in a child and a need to understand the child's whole situation, even though their own feelings may shake them to the core.
