Abstract | This article deals with a 16-year-old boy who is referred by his parents to address his academic decline and mood symptoms. He was diagnosed with attention-deficit/hyperactivity disorder (ADHD) 7 years ago, but his parents had opted not to have it treated. The patient has prominent inattention and distractibility, with complaints of internal restlessness. Grades have declined from Bs to Ds. On examination, in addition to ADHD symptoms, the patient reports being "down at times," although he does not manifest neurovegetative signs or symptoms. He also admits to smoking marijuana regularly for the past few months, with daily use of "at least a few hits" and "more on weekends." The authors discuss practical approaches to issues in pediatric pharmacotherapy before embarking on a pharmacological treatment plan. The focus is primarily on psychopharmocological management, and the responses from the clinicians are meant to provide examples of thought processes that may precede pharmocotherapy decision making.
