AbstractEmpathy is considered a significant factor in the physician-patient relationship. The current study examined the impact of a Longitudinal Integrated Clerkship (LIC) model on empathy and patient-centered attitude in medical students. Archival data were examined from 186 medical students at a medical college in the mid-Atlantic region of the United States. Students completed the Jefferson Scale of Physician Empathy-Student version (JSPE-S) and the Patient-Practitioner Orientation Scale (PPOS) at the beginning of each of their first three years of education, as well at the end of the third year and fourth year. Females had higher scores on empathy measures than males, but no difference was found between people-oriented and non-people-oriented specialties on measures of empathy. Empathy significantly decreased between the beginning of the third year of medical school and the end of the fourth year. However, unlike previous studies, empathy did not decrease between the beginning of the third year and the end of the third year. As the students participate in the LIC during their third year of training, it is possible that the model creates a buffer to the expected erosion of empathy. A strong positive correlation was found between empathy and belief in a patient-centered approach. Additional exploratory analyses further examined the relationship between LIC, gender, and specialty choice and the PPOS. The results indicate that future research comparing a traditional block clerkship to the LIC model could provide stronger conclusions regarding the impact of the LIC model on empathy erosion.
