AbstractThe York County (Maine) Transition Project was designed to improve postsecondary education and employment opportunities for learning-disabled students. Three survey forms for evaluating the impact of the project comprise this document. The first survey is a six-page questionnaire targeted to practitioners. It gathers data on: (1) the project's assessment process for identifying students' needs and developing transition plans; (2) usefulness of inservice training workshops; (3) development or improvement of linkages with school and community resources; (4) improvements in transition planning due to the project; and (5) the project's impact on employment opportunities. The second questionnaire is a student follow-up survey and gathers data on employment status, job characteristics, job satisfaction, services that were helpful in getting a job, plans for the future, and students' attitudes toward transition project services. The final survey form asks parents to provide information about their son or daughter's employment status, services utilized to acquire the job, services that could help in getting a job, importance of various elements of special education and vocational education, and opinions of the transition project.
