AbstractThis report describes and analyzes the transition process used at Savanna High School, Anaheim, California, and documents the unique contribution of the school's Independent Living Skills Transition Center. The report's seven sections focus on: (1) transition definitions and the need for a shared definition among agency personnel; (2) team membership and the individualized education program meeting; (3) transition team roles and issues impeding transition skills acquisition; (4) analyses of three model systems, depicting the dynamic exchange of student information among service providers, the networking process among transition team members, and the typical student paths through the school system and other services en route to adult community living; (5) subjective and objective evidence of student needs and program efficacy; (6) key elements of a team model; and (7) optimization of the transition process. Appendices contain survey forms and program evaluation forms used with school personnel, parents, students, and community agency representatives; a manual titled "Model Program and Guidelines for Occupational Therapist Training in Delivery of High School-Based Transition Services for Students with Disabilities," edited by Jeanne Jackson (Appendix B); and a study titled "Transition Needs Assessment of High School Students with Severe Disabilities and Their Parents and Teachers," by Florence Clark et al.
