Abstract | A "failsafe" technology is presented based on a new unified theory of needs assessment. Basically the paper discusses fault tree analysis as a technique for enhancing the probability of success in any system by analyzing the most likely modes of failure that could occur and then suggesting high priority avoidance strategies for those failure modes. It provides a logical, step by step approach to the identification of possible failure factors and the interactive effects of those factors which could result in a predetermined undesired event. The fault tree is so named because the completed graphic portrayal of logic gate related potential failure sequences takes on the outline form of a branching tree. (Author/RC) |
