Abstract | This training module is intended to assist families and service providers in planning the movement from home-based to center-based early intervention and preschool services for young children with deaf-blindness and other severe disabilities. The module's objectives for the trainee include recognizing the importance of agency collaboration for transition from early intervention services to preschool; demonstrating an understanding of the laws and how transition is addressed in the laws; and gaining information on structuring the coordination process according to recommended best practices. The module has 10 sections: (1) the introduction and pre-post test; (2) a section on the importance of collaboration for successful transition; (3) relevant legislation including Parts H and B of the Individuals with Disabilities Education Act and Head Start legislation; (4) samples of goals appropriate for meeting the transition requirements of the Individualized Family Service Plan; (5) suggested ways for gathering information and identifying needs; (6) a model for facilitating the transition meeting; (7) suggestions for recognizing and dealing with common barriers to interagency coordination; (8) definitions; (9) a resource list and bibliography; and (10) appended sample forms.
