AbstractThis dissertation investigated Howard University College of Medicine's medical students' perceptions concerning the factors that will influence their intended choices of specialty. The medical school is located in the Baltimore-Washington, DC Metropolitan region, which is suitable for investigation because it ranks second nationally in terms of the concentration of healthcare professionals in the United States. The study was prompted by four major problems: (1) physician shortage in the United States, especially in the area of primary care; (2) health professional shortage in medically underserved areas/populations; (3) physician workforce diversity; and (4) underrepresented minorities in medical education. Based on these issues, the following major research questions were probed: (a) What factors do Howard University medical students believe cause physician shortage in the United States, especially in the area of primary care? (b) What factors do Howard University medical students believe cause health professional shortage in medically underserved areas/populations? (c) Do Howard University medical students believe there is physician workforce diversity? (If yes, what are the reasons? If no, what are the reasons?) Given these questions and findings from relevant literature on the topic, the following hypotheses were tested in this study: H1: Howard University medical students' perceptions about physician shortage in the United States, especially in the area of primary care, are influenced by their income levels, amounts of debt, life styles, and social identities; H2: Howard University medical students' perceptions about health professional shortage in medically underserved areas/populations are influenced by their attraction to advantaged communities; and H3: Howard University medical students' perceptions about physician workforce diversity are influenced by their social identities. To test these hypotheses using Rational Choice Theory and quantitative methodology, a one-group post-test only design, was utilized. A convenience sample of 216 medical students in their pre-clinical and clinical years of education at Howard University College of Medicine responded to a self-generated survey. Using document analysis from previous studies and relevant reports, the survey data was quantitatively analyzed at the univariate, bivariate, and multivariate levels by using the Statistical Package for the Social Sciences (SPSS). With a P-value and a = 0.05 significance level, the research findings showed partial significance in the social and economic class, professional obligation to care for the uninsured and under insured, placed were they live and work, and medical student commitment to political issues and activities. Finally, recommendations were based on the aforementioned finding so as to address the physician shortage, especially in primary care.
