| Abstract | A process is outlined for developing an evaluation form that meets the following criteria: is relevant at local, state, and federal levels; short; objective; usable; statistically manageable; analytic; and fiscal. With standards in mind, the next step is to specify the kinds of information needed to meet state and federal requirements. Once the types of information needed are specified, determination is to be made of whether or not this information is available in the department. If not, the development of a data collection form may be indicated. The evaluation form is evolved through eleven stages: (1) review past evaluations for usable items and data; (2) identify additional observable items; (3) identify hard to observe items; (4) develop categories of acceptable evidence; (5) arrange items on a one-page form; (6) revise and field test the form; (7) revise the form; (8) field test the form on a new sample and make further revisions; (9) categorize items into an accepted model; (10) make any final revisions; and (11) develop summary pages for statistics. A number of examples are presented to illustrate the stages involved in constructing and revising an evaluation form. (BW) |
